Aims of the service
Aims of the Hospice at Home serviceThe aim is to offer patient-centred care and comprehensive home support, strengthened by a range of other Hospice services, such as social work, chaplaincy and counselling.
ReferralsIf you think you could benefit from the services of Hospice at Home please do contact us. You can also discuss your referral with your GP or District Nurse. Referrals are also made by Macmillan Nurses, hospital consultants, hospital doctors, nurses and all allied health professionals including nursing and care homes. Links with Primary Care TrustThe service works in partnership with the two local Primary Care Trusts of Hastings & St Leonards and Bexhill & Rother. We work collaboratively with our colleagues in primary care to ensure patient support is tailored to meet the needs of each individual. All care is co-ordinated with your GP or District Nurse. We will support you and your family if requested to do so, alternatively we provide support to all healthcare professionals by offering holistic care to you in your home environment. Additional supportVolunteersA specially selected team of volunteers are an important part of our team. The Volunteer service offers help with a wide range of tasks, such as shopping, preparing light meals, dog walking, collecting library books etc Complementary therapiesYou or your family / carer may wish to receive some complementary therapy in order to alleviate any physical or psychological symptoms. The Sara Lee Trust can provide a trained therapist, without charge to visit and treat you in the comfort of your own home. Therapies include aromatherapy and reflexology. Your Hospice at Home nurse will discuss with you whether you need any additional support. Once in the care of St Michael’s Hospice we can offer:
