Emotional support and advice

Our Patient Family Support Worker is able to provide emotional support and advice on a wide range of practical matters, such as:

Benefit entitlement
Facilitating packages of community care following a discharge from the Hospice
Liaison with Social Services and other agencies to ensure that patients and their families get the practical help they need

Referral to the Patient Family Support Worker can be made through any member of the clinical team.
